Five Wishes Online - Five Wishes has become America's most popular living will because it is written in everyday language and helps start and structure important conversations about care in times of serious illness.

We are making Five Wishes available to our community. Please see below for two ways to access Five Wishes from The Denver Hospice.

  1. To complete your Five Wishes online, please email us at and we will send you an access key which you can enter here. Your info will be retained online and you can return to completing your Five Wishes at your convenience.

  2. To request a copy of Five Wishes, please click here to fill out an online form. Five Wishes can be picked up at our Administrative Offices or mailed to your residence.


MOST - The Medical Orders for Scope of Treatment form is a 1-page, 2-sided document that consolidates and summarizes patient preferences for key life-sustaining treatments including: CPR, general scope of treatment, antibiotics, artificial nutrition & hydration.

The Conversation Project - The Conversation Project is dedicated to helping people talk about their wishes for end-of-life care. It’s not easy to talk about how you want the end of your life to be. But it’s one of the most important conversations you can have with your loved ones.

Life Quality Institute - Life Quality Institute provides palliative care education in Colorado for physicians, nurses, physician assistants, medical students and caregivers.

Hospice Journey - Hospice Journey provides free personal websites for people facing terminal illness. Hospice Journey is offered by The Samvara Project, a 501(c)(3) non-profit organization, in collaboration with hospice organizations throughout the United States.
