PERSONAL CARE ASSISTANCE, 24/7 – The Denver Hospice employs highly skilled Certified Nursing Assistants who are available whenever you need them – including weekends. Nursing assistants help with ongoing daily care needs such as bathing, changing bedding, dressing, providing meals and daily walks.

 

 

A TEAM OF EXPERIENCED PROFESSIONALS – Our team of experienced nurses, administrative professionals, CNAs, physicians, social workers and chaplains are on call 24-hours a day, seven days a week. We have the depth of services and a full staff to fulfill a variety of palliative care, grief counseling and full service hospice needs.

PERSONALIZED HOSPICE SERVICES – How someone wants to receive hospice services can vary greatly from one person to the next. That’s why we work closely with patients and the facility caring for them, and design an appropriate delivery of services that matches unique sensitivities and needs.

RESPOSE TO YOUR NEEDS – We give you our guarantee that within one week after admission, we’ll have a conference meeting scheduled between our team, the patient and family, and care facility personnel. Being responsive is a top priority at The Denver Hospice.

TIMELY ADMISSION – Once we get your referral, we send out an admission team within a two-hour time frame. And you can always speak with our trained admissions staff immediately.

OPEN CHANNELS OF COMMUNICATION – When patients are receiving hospice care, their situation can change from moment to moment. That’s why we recognize the importance of immediate and ongoing communication with patients, the patient’s family, and care providers, at all times.

WORDS INTO ACTION – We develop a collaborative plan of care coordination with you and revise it frequently with your input. We review this plan word by word, with all parties involved.
